Seeking web-based assessment software

I'm seeking a web-based assessment software similar to Assessment Generator but with more features.  Namely, when someone takes the assessment and scores in a certain percentile, the person can be referred to a particular page of resources that will enable that person to move forward to the next step.

Also, ideally, the person's progress is mapped out in a graphic interface.  That is, If there are 5 steps that need completion and the assessment indicates that the person is qualified only up through step 2, the program would create a completion meter of sorts showing that the person is at Step 2 and needs to go to a certain page of a website to get the next steps.
